In order to redeem loyalty points for free* products
(30-day supply only) your account must be in good standing with
no bills more than 60 days past due. Customer is responsible to
mail in redemption coupon with $5.00 processing fee (per free* product)
when at least 20 points are accumulated. Loyalty Points are not
redeemable for cash, not transferable, expire after 12 months, and
cannot be used as payment. Payments received on seriously delinquent
bills do not earn loyalty points. No loyalty points are granted
for free products or discounted products. Discount club members
accrue 2 points per paid product, instead of 5. Additional processing
fees will apply to free* products redeemed by customers outside
the USA. After 4 months, unapplied processing fee payments will
be applied to open orders, if any. Tabak's Health Products reserves
the right to modify or terminate the Loyalty Rewards Program at
any time without notice. Upon termination of the program, existing
loyalty points will be valid for 6 months.
